Code Silver - Hostage, Active Shooter, Threat of Violence PURPOSE: Code Silver is established to facilitate the appropriate response to a hostage/active shooter or threat of violence situation. The following plan outlines the actions to be taken by Chatham Hospital in the event of this/these situation(s). PROCEDURE: It is important to note that the first five to ten minutes in a hostage situation are the most dangerous. Staff in the area(s) affected by the hostage situation will perform as necessary. When a hostage situation occurs, the following procedure should be followed: Notify security (or maintenance if security is not in hospital) by dialing 30-00 and paging “Code Silver” 3 times; announce known location(s). Notify law enforcement by calling 911. After the person receiving the call or observing the active shooter/hostage or violent situation provides the details, they will become the Incident Commander (until this duty has been delegated to an individual more appropriate or more trained) and will make the necessary decisions, issue the orders and prepare for the arrival of assistance. The Small Conference Room will become the Command Center. The Incident Commander will be responsible for notifying the Maintenance Supervisor and the Security Guard to standby with master keys, who will also secure and stand by with floor plans of the hospital. There are boxes locked outside the ED ambulance doors and on the back loading dock at the stairs that are accessible by Siler City Police Department. They contain master keys to the hospital. Staff in the affected location will attempt to evacuate the area as quickly and safely as possible. If not in the affected area; DO NOT ENTER. Staff will close the fire doors surrounding the affected area. Security or maintenance will be responsible for controlling access to the affected area until law enforcement arrives. No one is to enter the hospital. All entry doors to the hospital are to be locked and monitored by maintenance or security until law enforcement arrives. Upon arrival of the law enforcement officer, the director(s) or designee(s) of the area(s) affected will advise the officer of all available information regarding the location of the hostage taker, number of hostages, etc. DO NOT make promises or threats. DO NOT identify yourself by title – first name only (i.e. Sue, Mary, etc.). Once Law Enforcement arrives and becomes apprised of the situation, they will become the Incident Commander; the hospital will also have an Incident Commander, resulting in a Unified Command. Staff outside the area affected will perform as necessary in the event of a hostage/active shooter/threat of violence situation. Staff will continue with normal duties. However, staff will not attempt to enter the hostile area. Staff will be prepared to evacuate if required. Active Shooter: If staff hear shots being fired or see an individual start shooting a firearm, the following actions should be taken Exit the building immediately Instruct anyone you encounter to exit the building immediately Notify the police department by calling 911 and/or maintenance/security and provide the following information: - Your name - Location of the incident - Number of shooters - Description of the shooter(s) - Description of the weapons involved - Number of persons with you and any injuries - Your current location If you are unable to exit the building, the following actions should be taken: Lock yourself in the nearest office or closet Cover the windows Keep quiet and act as if no one is in the room DO NOT answer the door If possible, notify the police department by calling 911 and/or maintenance/security and provide the following information: - Your name - Your location (be specific as possible) - Number of shooters - Description of shooters - Description of weapons involved - Number of persons with you and any injuries Do not leave your location until ordered to do so by Law Enforcement. If you are directly involved with patient care the following actions should be taken: If possible, close and barricade the doors to the unit Close all patient room doors Try to move all ambulatory patients to a secured area of the unit Keep as quiet as possible If possible, notify the police department at 911 and/or maintenance/security and provide the following information: - Your name - Your location (be specific as possible) - Number of shooters - Description of shooters - Description of weapons involved - Number of persons with you and any injuries Do not leave your location until ordered to do so by Law Enforcement. Threats of violence in the workplace: If you observe an individual making threats of violence or threats of kidnapping against patients, visitors, or staff: Remain calm Call 911 and/or maintenance/security or use alternate notification method immediately Make no sudden movements Attempt to calm down the individual As safely as possible alert others in the area to the threat If you receive a phone call regarding the threat of violence or a kidnapping/hostage situation: Keep the caller on the line Get as much information as possible Call 911 and/or maintenance/security or use alternate notification method as soon as possible If you receive a ransom note or a note indicating a threat of violence: Call 911 and/or maintenance/security or use alternate notification method immediately Minimize handling of the note Secure the note for Police INCIDENT COMMAND: The staff person who is witness to or receives notification of a hostage/active shooter or threat of violence situation is the designated Incident Commander and is to carry out the duties and responsibilities of this role until Incident Command can be delegated to an appropriate replacement. After calling 911 and ensuring the safety of staff, patients and visitors, the hospital President or administrator on call should be notified as soon as possible. Law Enforcement, upon arrival, will be the Incident Commander for the situation but an Incident Commander must remain in charge for Chatham Hospital. Law Enforcement is responsible for determining a staging area. The Incident Commander will perform as necessary in the event of a hostage, active shooter or threat of violence situation: Announce Code Silver by dialing 30-00 and announcing Code Silver and general location three times. Contact 911 Set up a Command Center in a safe location (Small Conference Room if possible). Log onto Fast Command System via the Chatham Hospital Intranet, initiate and follow appropriate protocols and instructions for Incident Commander. The Incident Commander will designate a Safety Officer, a Liaison Officer and a Public Information Officer, along with Section Chiefs for Planning, Logistics, Administration and Operations according to Chatham Hospital's Emergency Operations Plan. All designees will have duties and instructions for their respective roles on Fast Command. The Incident Commander will conclude Code Silver when advised to do so by law enforcement. Recovery: The Incident Commander is responsible for terminated emergency management processes and initiating the recovery process. Employee Health and Human Resources will coordinate a staff incident stress debriefing and any follow-on counseling. The Hospital President will coordinate resumption of any curtailed services. A facility damage assessment will be conducted by the Maintenance Department and schedule repair work prior to reoccupying damaged areas of the hospital. The Chief Financial Officer will prepare and process all authorized claims. The Incident Commander will schedule a debriefing and conduct a critique of the response. All supplies, drugs, water, food, linen and fuel will be replenished. |
